Joint Pain: Why Do My Joints Hurt? Causes, Tests and Warning Signs

Joint pain (arthralgia) is extremely common. It can affect one joint or many, be acute or chronic, and arise from infection, inflammation, degenerative disease, crystals, injury or autoimmune conditions. The pattern of joint involvement, timing and associated features provide important diagnostic clues.

Common Causes of Joint pain

1. Viral Infections (Dengue, Chikungunya)

Both dengue and chikungunya are mosquito-borne and common in India. Dengue causes severe body and joint pain with high fever, headache, pain behind the eyes and rash. Chikungunya is characterised by high fever and often debilitating joint pain — particularly affecting hands, wrists, ankles and feet — that can persist for months.

2. Osteoarthritis

Degenerative joint disease affecting weight-bearing joints (knees, hips, spine) and hands. Causes joint pain worsening with activity, morning stiffness lasting less than 30 minutes, bony enlargement and reduced range of motion. Common after 45 years.

3. Rheumatoid Arthritis

An autoimmune inflammatory arthritis affecting multiple small joints symmetrically — especially hands, wrists, feet. Causes prolonged morning stiffness (>1 hour), swelling, warmth and tenderness. RF, anti-CCP, CRP/ESR, CBC and joint X-rays are key investigations.

4. Gout

Caused by uric acid crystal deposition. Often presents as sudden severe pain in one joint — most classically the big toe (podagra) — with redness, warmth and swelling. Episodes are triggered by alcohol, purine-rich food, dehydration, diuretics or fasting. Serum uric acid and synovial fluid analysis are the key tests.

5. Reactive Arthritis

Joint pain developing 1–4 weeks after an infection (usually gastrointestinal or urogenital). May affect large joints asymmetrically. May be associated with eye inflammation or urogenital symptoms.

6. Psoriatic Arthritis

Joint inflammation associated with psoriasis. Can affect any joint. May present before skin disease becomes apparent. Nail pitting and dactylitis (sausage digits) are clues.

7. Septic Arthritis

A joint infected by bacteria — causes severe pain, swelling, warmth and fever. This is a medical emergency requiring urgent treatment. A single hot, red, painful joint should always raise suspicion.

8. Systemic Lupus Erythematosus (SLE)

Autoimmune disease causing joint pain (usually non-erosive), rashes (butterfly rash), fatigue, fever, kidney involvement and other organ features. ANA is the initial screening test.

9. Hypothyroidism

Can cause muscle and joint pain, stiffness and sometimes joint effusions. Associated with fatigue, weight gain, cold intolerance. TSH is the key test.

10. Vitamin D Deficiency (Severe)

Severe Vitamin D deficiency can cause bone pain and proximal muscle weakness that patients sometimes describe as joint pain. A 25-OH Vitamin D level can clarify.

Seek urgent medical attention if you experience any of the following:

  • 🚨 Hot, red, swollen single joint with fever — possible septic arthritis (surgical emergency)
  • 🚨 Joint pain after a recent penetrating injury
  • 🚨 Sudden severe joint pain in a person taking anticoagulants (haemarthrosis)
  • Persistent joint swelling lasting more than 6 weeks
  • Joint pain with prolonged morning stiffness (>1 hour) — possible inflammatory arthritis
  • Joint pain with significant skin rash, fever or weight loss
  • Joint pain with reduced kidney function or blood in urine
  • Joint pain that is rapidly worsening with fever and general deterioration

Can dengue or chikungunya cause joint pain?

Yes. Both are mosquito-borne and common in India. Chikungunya in particular can cause severe, debilitating joint pain that may persist for months. Dengue causes myalgia and arthralgia with fever.

What is the difference between osteoarthritis and rheumatoid arthritis?

Osteoarthritis is degenerative and mainly affects older patients, is worsened by activity, and morning stiffness lasts less than 30 minutes. Rheumatoid arthritis is autoimmune, can occur at any age, affects many joints symmetrically, and morning stiffness typically lasts more than 1 hour.

Is a high uric acid level always gout?

No. Many people with high uric acid never develop gout. Conversely, serum uric acid can sometimes be normal during an acute gout attack. The diagnosis is clinical, supported by uric acid levels and sometimes synovial fluid analysis.
