Total Protein and Albumin: Normal Ranges and What the A:G Ratio Means

Total protein measures albumin plus globulins in serum. Normal total protein is 6.4–8.3 g/dL, albumin 3.5–5.2 g/dL and the albumin-to-globulin ratio 1.1–2.5. A low albumin points to liver disease, kidney loss, malnutrition or inflammation, while a reversed A:G ratio should prompt serum protein electrophoresis.

Serum protein has two components. Albumin, made exclusively by the liver, makes up just over half of the total and is responsible for holding fluid inside blood vessels through oncotic pressure and for transporting calcium, hormones, bilirubin and many drugs. Globulins, the remainder, include immunoglobulins made by plasma cells and a range of transport and acute-phase proteins.

Globulin is not measured directly — it is calculated as total protein minus albumin — and the albumin-to-globulin ratio derived from the two carries real diagnostic weight. A reversed ratio, where globulin exceeds albumin, occurs when albumin falls in liver disease or protein loss, or when globulins rise in chronic infection, autoimmune disease or a plasma cell disorder.

Albumin has a half-life of about 20 days, so it changes slowly and reflects chronic rather than acute nutritional or hepatic status. It is also a negative acute-phase protein — it falls during any significant inflammation regardless of nutrition, which is why a low albumin in an unwell patient is more often a marker of illness severity than of poor diet.

Q: What is the normal range for total protein and albumin?

A: Total protein 6.4 to 8.3 g/dL, albumin 3.5 to 5.2 g/dL, calculated globulin 2.0 to 3.5 g/dL and an albumin-to-globulin ratio of 1.1 to 2.5. Albumin falls slightly with age and falls in pregnancy through dilution.

Q: What does low albumin mean?

A: Low albumin has four main explanations: the liver is not making enough, as in chronic liver disease; it is being lost, through the kidneys in nephrotic syndrome or through the gut; intake or absorption is inadequate; or inflammation is suppressing production. It is also a general marker of illness severity in hospitalised patients.

Q: What does a reversed A:G ratio mean?

A: A reversed ratio means globulins exceed albumin, which happens either because albumin has fallen or globulin has risen. In an adult over 50, particularly with anaemia, bone pain, kidney impairment or a very high ESR, it should prompt serum protein electrophoresis to look for a monoclonal protein.

Q: Do I need to fast for a total protein test?

A: An 8-hour fast is preferred and it is usually collected with a liver panel. Just as important is sitting for five minutes before the draw and keeping the tourniquet brief, since posture and prolonged tourniquet application can each shift protein values by up to 10%.

Q: Can low albumin be corrected by eating more protein?

A: Only when poor intake is genuinely the cause. If albumin is low because of liver disease, kidney loss or inflammation, increasing dietary protein will not raise it — the underlying condition has to be treated. In liver disease and kidney disease, protein intake actually needs to be managed carefully rather than simply increased.

Q: What does high globulin mean?

A: A high globulin can be polyclonal, from chronic infection, chronic liver disease, autoimmune disease or HIV, or monoclonal, from a plasma cell disorder such as myeloma or MGUS. Serum protein electrophoresis distinguishes the two, and it is the single most useful next test when globulin is unexpectedly high.

Q: Why does albumin matter for interpreting calcium?

A: About half of blood calcium is bound to albumin, so a low albumin lowers total calcium without any true deficiency of the active ionised fraction. Corrected calcium adds 0.8 mg/dL for every 1 g/dL the albumin sits below 4 g/dL, and using it prevents a lot of unnecessary calcium supplementation.

Q: Is albumin a good marker of nutrition?

A: It is a weaker marker than commonly assumed. Its half-life of about 20 days means it responds slowly, and it falls during any inflammation regardless of what someone is eating. Prealbumin responds faster, and a proper nutritional assessment relies on weight trend, intake history and clinical examination as much as on any blood test.

Q: What causes high total protein?

A: Most often dehydration or a prolonged tourniquet, both of which concentrate the sample. Genuine elevation comes from raised globulins — chronic infection, autoimmune disease, chronic liver disease, or a monoclonal gammopathy. The albumin and globulin values, not the total, tell you which.

A: When globulin is raised or the A:G ratio is reversed, particularly in an adult over 50; when there is unexplained anaemia, bone pain, kidney impairment or hypercalcaemia; and when the ESR is very high with a normal or near-normal CRP. It is the test that separates a harmless polyclonal rise from a monoclonal protein.
