Uric Acid Test: Normal Range, Causes of High Uric Acid and What to Do

Uric acid is the end product of purine breakdown, cleared mainly by the kidneys. Normal is 3.5–7.2 mg/dL in men and 2.6–6.0 mg/dL in women. Levels above 7 mg/dL raise the risk of gout and kidney stones, but most people with high uric acid never develop symptoms and do not automatically need medication.

Purines, found in body cells and in certain foods, break down through xanthine into uric acid. Humans lack the enzyme uricase that other mammals use to degrade it further, so uric acid is our end product and must be excreted — about two thirds by the kidneys and one third through the gut.

Levels rise either because production goes up (high cell turnover, alcohol, fructose, purine-rich food) or, far more often, because renal excretion falls (kidney impairment, diuretics, dehydration, insulin resistance). Under-excretion accounts for roughly 90% of hyperuricaemia in practice.

Above roughly 6.8 mg/dL, uric acid exceeds its solubility in body fluid and can crystallise as monosodium urate in joints, causing gout, or as uric acid stones in the urinary tract. Crucially, most people with a high level never crystallise anything — asymptomatic hyperuricaemia is common and is usually not treated with drugs.

Q: What is the normal uric acid level?

A: 3.5 to 7.2 mg/dL in adult men and 2.6 to 6.0 mg/dL in adult women. Women's levels rise towards male values after menopause. If you have established gout, the treatment target is lower — below 6.0 mg/dL, and below 5.0 if you have tophi or frequent attacks.

Q: What are the symptoms of high uric acid?

A: Most people with high uric acid have no symptoms at all. When symptoms occur, they are sudden severe pain, swelling, warmth and redness in a joint — classically the base of the big toe, often starting at night — or the passage of kidney stones. High uric acid does not cause general body aches or fatigue, despite the widespread belief that it does.

Q: What foods should I avoid with high uric acid?

A: The strongest offenders are alcohol, particularly beer and spirits, sugar-sweetened drinks and anything with high-fructose content, organ meats such as liver and kidney, red meat and certain seafood including prawns and sardines. Dals, pulses and vegetables raise uric acid far less than commonly believed and do not need to be eliminated.

Q: Does high uric acid always mean gout?

A: No. Asymptomatic hyperuricaemia is common and most people who have it never develop gout. Gout is diagnosed by the clinical picture of acute crystal arthritis, ideally confirmed by finding urate crystals in joint fluid — not by a blood level alone.

Q: What does a uric acid of 7.5 mean?

A: 7.5 mg/dL is above the reference interval for both men and women and above the point at which urate can crystallise. If you have never had joint pain or a stone, it usually calls for weight management, less alcohol and fructose, review of any diuretic, and repeat testing — not immediate medication. If you have had gout, it means treatment needs intensifying.

Q: How can I reduce uric acid naturally?

A: Lose weight gradually, cut alcohol and sugar-sweetened drinks, stay well hydrated, reduce red meat and organ meat, and treat insulin resistance through diet and exercise. Low-fat dairy and vitamin C have modest lowering effects. Crash dieting and fasting raise uric acid and can trigger an attack, so weight loss should be gradual.

Q: Do I need to fast for a uric acid test?

A: An 8–12 hour fast is preferred for consistency, especially when monitoring treatment. Avoid alcohol for 24 hours and avoid an unusually meat-heavy meal the night before, as both can raise the result independently of your usual level.

Q: Why is my uric acid normal during a gout attack?

A: Uric acid frequently falls during an acute attack because urate is being deposited into the joint and the inflammatory response increases its excretion. A normal level during an attack does not rule out gout. The right time to measure your true baseline is about two weeks after the attack has fully settled.

Q: Should asymptomatic high uric acid be treated with medication?

A: Usually not. Current rheumatology guidance does not recommend routine urate-lowering drugs for people with high uric acid who have never had gout or stones. Exceptions include very high levels, planned chemotherapy, and some cases of chronic kidney disease. This is a decision for your doctor based on your full picture.

Q: Can high uric acid damage the kidneys?

A: Very high levels can cause uric acid stones and, in acute tumour lysis, acute kidney injury. Whether modestly raised uric acid causes progressive kidney disease remains debated — it may be more a marker of reduced kidney function than a cause of it. Kidney impairment itself raises uric acid, so the relationship runs both ways.
